The Title indicates the type of Navigation equipment required to "shoot" this particular approach, the Airport Identifier, Airport location, Airport Name . and Intended runway.

In this particular example a VOR or GPS is required, it is the approach for Runway 25 at Santa Barbara Municipal in Santa Barbara, CA.

If there is NO runway listed, then the approach is a CIRCLING approach and is designed to allow the aircraft the capability to fly around the airfield visually to land on a particular active runway. Normally, a letter such as A, B, C will follow the Navaid. This tells you that the approach is VOR approach A, or B etc.. in case of multiple circling approaches for the same airfield using the same navaid(s).
